Accurate and timely reimbursement starts with an accurate and complete OASIS form. Either the RN or qualified therapist use this form to document updates to the comprehensive assessment. This assessment integrates the OASIS data sets for recertification (follow-up) and other follow-up (significant change in condition). It also includes the information areas needed for the CMS Plan of Care 485 or equivalent. It should be completed to provide guidance for developing the Home Health Certification and Plan of Treatment for the next episode of care and for revising applicable care plan documents.
